Forms Download Page

There are five forms to fill out before the charter.  Your trip-arranger receives these in his trip-arranger package.  If you haven't received your forms or have misplaced them you can download them from this site.  There are four diving forms and one boat specific form.   You will need to fill out and sign the TDI Medical Statement as part of the required forms.  If you decide you need a physical examination, you can download the four page "Guidelines for Recreational Scuba Diver's Physical Examination" which can be referenced by your physician.  Click on any form to open it in Adobe Acrobat Reader.  You can then print it using the print icon in the Acrobat Reader tool bar.  If you need to download the free Adobe Acrobat Reader click here.
